Prepare for Your Visit
To prepare for your visit, we recommend bringing the following, if applicable:
Previous medical records
Recent doctor visit summaries
Lab results from the past 1–2 years
Imaging reports (X-rays, MRI, CT scans, Ultrasound)
Current medication list
Insurance card and photo ID
Allergy information
By providing this information beforehand, we can get a better understanding of your medical background and ensure your first appointment at our primary care clinic is informative and efficient.
Reschedule a Visit
If you need to reschedule your appointment, please provide us with at least 48 hours' notice. This allows us to offer the appointment slot to another patient in need.
